New Carrier Setup Requirements

• Certificate of Insurance (Cargo and Liability). DANA Transportation will need to be listed as a certificate holder with your insurance company.

• W9

• Authority

• If a factoring company is used, the factoring company NOA (Note of Assignment) is required

• Complete pages 3, 9, & 15 of carrier packet (ref link below)
  • Carrier Packet
